To help the MN Department of Health is providing $100 Visa Gift Cards for people who receive their first COVID-19 vaccination between July 30th to August 15th, 2021.
You must be 18 or older to receive the $100 incentive. If you are between the age of 12 and 17, please have your parent or guardian fill out this form.
